How to Write a Work Request

To report an issue in the library, order keys, and/or request an estimate, please fill out a Work Order Request form.

Above is an image of the Work Order Request.  Below is a description of the information needed for each field within the Work Order Request:

  • Tracker: For work order requests, ensure that the tracker is set to Maintenance Requests.
  • Subject: Similar to an email subject line, enter a succinct description of the request you are about to make that could be easily identified in a list or when searched.
  • Description: Explain the needs of the request. Please remember to be as specific as possible to expedite the process.
  • Status: New is the default status. This will be adjusted as members of the Facilities & Events team begin executing the request.
  • Start Date: Defaults to the date entered.
  • Priority: Normal is the default. Low, High, or Immediate are also options in this field. If high or immediate is chosen, please also contact a member of the Facilities & Events team directly.
  • Due Date: If the request has a specific date it needs to be done by or on, please chose the date accordingly for consideration.
  • Library: Choose the appropriate library facility the request is pertaining.
  • Cost: Facilities & Events team members use this to keep record of the cost associated with work directly billed to the Libraries.
  • Room #: Please use the specific, numeric room number for the area the request is pertaining. Maps are located to the right to verify specific numeric room numbers.
  • Assignee: Facilities & Events will assign the request to a member of the team for follow-up and follow-through.
  • Work Order #: Facilities & Events will enter the assigned work order number given to the request by PFD.
  • Account #: If work is to be charged to a specific budget account number. All requests for estimates require a budget account number.
  • Category: Maintenance is the default. Choose the appropriate category for your request. Other options include Room Key/Door Access, Events, and Estimate. 
  • Files: Upload any additional files that could be of use to expedite the process (i.e. pictures of the maintenance issue, sketches of designed renovation for estimate)

Final step is to submit the work request. Allow for 1-2 business days to hear from a member of the Facilities & Events team.

Access & Security

Miami ID Card

Miami University ID cards are used for admission to various University facilities and activities and as a form of payment if you so choose. The lending, borrowing, altering, or duplicating of an ID is strictly prohibited. Unauthorized use of a Miami ID may result in criminal charges and university disciplinary action. All library staff are granted access to necessary library facilities as part of the on-boarding process. If you lose your Miami ID, please contact Card Service Center immediately to procure an updated card and officially deactivate the lost card.

Security Codes

King Library, B.E.S.T. Library, Peabody Archives, and Special Collections are all monitored by a third party security system. If a security code is necessary for your employment one is also provided during the on-boarding process. If you lose or forget your security code, please contact Mike Smith.

Access & Schedules

Access to and schedules for library facilities are managed by the Facilities and Events team in collaboration with Card Service Administration. Schedules are entered each academic year and updated for changes at the beginning of each semester. If you encounter difficulties with your access or have a question regarding a facilities schedule, please write a work order to alert the team; your unique ID and specific details will be necessary to troubleshoot errors.

Floor Plans

Each library name links to general information related to the building it is housed in. Each floor under a designated library links to the floor plan with floor specific information. All links require unique ID and password to access Physical Facilities FM Interact system.
