Adult User Waiver

Makerspace Use Agreement and Release of Liability
All users of the Makerspace must complete a liability waiver prior to their use of the space.

Minor User Waiver

Makerspace Use Agreement and Release of Liability for Minor Users
Users under 18 years of age should have their legal guardian complete this form. When you turn 18, you will need to fill out our Adult User Waiver.
